At about 15:30, the operator Li Zuokun went to the middle of the accident well and hit the quick-drying cement at the DN250 pipe opening with a hammer. At this time, Li Ping was in charge of handing tools next to the accident well. He Guoqiao and He Hu were working near the accident well. When Li Zuokun knocked a large mouthful of the quick-drying cement at the DN250 pipe port, a large amount of sewage rushed out, and Li Zuokun fainted at the bottom of the well. Upon seeing this, Li Ping immediately called He Guoqiao and He Hu to come to rescue.
After He Guoqiao arrived, he tied a knot on his left hand with a rope and went down to rescue the people. After reaching the bottom of the well, He Guoqiao also fainted quickly. Other people in Inoue tried to pull the rope tied to He Guoqiao's hand to rescue him, but because the rope was loose, He Guoqiao could not be pulled up. Immediately, He Hu took the rope and tied it to his waist and went down the well to rescue people. After reaching the bottom of the well, He Hu also fainted. The accident well has 4 sewage pipes connected. Before the incident, 2 DN400 sewage pipes were sealed with air bags, 1 DN250 sewage pipe was plugged with quick-drying cement, and 1 DN500 sewage pipe was connected with the corresponding The adjacent sewage well W1 is connected, and the bottom of the W1 well is lower than the bottom of the accident well, and a submersible pump is used to pump water from the sewage well W1 to maintain the low water level in the well. No water flows into the accident well. Therefore, there is basically no water in the well.
2. Excluding the possibility of accidents caused by falling from heights, object hits, electric shocks, etc.
After inspection, the bodies of the three deceased showed no obvious signs of falls or bruises, and only He Guoqiao had bruises on the head. The accident well was about 3.28 meters deep. Li Zuokun lost consciousness before falling to the bottom of the well and worked in the middle of the well, not more than 2 meters from the bottom of the well. The only electrical equipment on the job site on the day of the accident was a submersible pump for pumping water from the W1 sewage well. The electricity for the submersible pump is taken from an electric cabinet equipped with a leakage protection switch in the community. At the time of the incident, there was basically no water in the accident well. The deceased was wearing plastic half-length waterproof trousers and shoes when working, and was not in contact with water, which ruled out the possibility of electric shock.
3. The deceased was identified as hydrogen sulfide gas poisoning and secondary drowning and suffocation. The accident well has 3 sewage pipes connected with the community sewage pipe network. As Li Zuokun excavated the quick-drying cement with the DN250 nozzle, water flowed into the accident well. The water flowing into the accident well came from domestic sewage and septic tank sewage in the F/6 well.
(2) High concentrations of sulfide were detected in the F/6 sewage well connected to the accident well through a DN250 pipe.
The accident investigation team entrusted Guangdong Weizhong Testing Technology Co., Ltd. to conduct an investigation on the F/6 connected to the accident well on November 22, 2019. /6 The water and gas in sewage wells and septic tanks were tested. The test results showed that different concentrations of sulfide were detected in the accident well and its connected F/6 sewage well and septic tank water bodies. Among them, the F/6 sewage well had the highest concentration of sulfide in the water body, reaching 3.60mg/L, which exceeded the " "Groundwater Quality Standard" (GB/T14848-2017) stipulates the Class V groundwater standard (>0.10mg/L) 36 times. The sulfide-containing water body is closed underground, Degraded under semi-closed anaerobic conditions or under the action of sulfate-reducing bacteria, it is easy to decompose to produce hydrogen sulfide to form a limited space working environment containing hydrogen sulfide.
(3) High concentrations of hydrogen sulfide were detected in F/6 sewage wells and accident wells
On November 23, 2019, the accident investigation team was in the process of simulating accidents in accident wells and F/6 sewage wells connected through DN250 pipes. A multifunctional portable detector was used to monitor the hydrogen sulfide gas in the accident well. The changes in the on-site monitoring hydrogen sulfide concentration data are as follows: the DN250 tube was pulled out to block the airbag at 16:7 pm, the sewage in the F/6 sewage well flows to the accident well, and the hydrogen sulfide concentration was at the background value from 16:7 to 16:20. Rising rapidly to reach the upper limit of the detector's range (100ppm), from 16:8 to 16:11, the hydrogen sulfide concentration continued to exceed the upper limit of the detector's range (100ppm) and remained in an over-range state; after 16:12, The hydrogen sulfide concentration began to fall within 100 ppm and fluctuated between 3.1 ppm and 33.2 ppm. After the detector was taken out of the well, the instrument reading quickly dropped to the background level.
Through the simultaneous monitoring of the accident well pipe network blockage reduction operating environment and hydrogen sulfide concentration, it was determined that the hydrogen sulfide concentration in the operating environment of the three deceased persons was very likely to reach an immediate life-threatening or health-threatening concentration (≥100ppm), far exceeding the "Workplace Hazardous Factors Occupation" Exposure Limits Part 1: Chemical Harmful Factors" (GBZ2.1-2007) stipulates the maximum allowable concentration in the workplace (10mg/m3, about 6.66ppm).

(2) The indirect cause of the accident:
1. The construction unit failed to perform the main responsibility for production safety according to law. Qiaoyi Company collected management fees for the project, but failed to perform safety management duties in accordance with laws and regulations and the construction contract; Huang Suhui did not conduct safety education and training for temporarily hired construction personnel, and did not provide compliance monitoring and ventilation equipment as required. The development of the construction field in the whole district is limited. The investigation and remediation of space operations is not in-depth, and the safety training for industry employees is not in place. None of the companies involved have participated in any meeting on the inspection and remediation of operations in limited space, have not received any documents on the inspection and remediation of operations in limited space, and have not participated in any safety education and training organized by government departments.
4. The territories have not strictly implemented the supervision responsibility
